WHAT TO TURN IN:

Completed Application, along with the following:
  • Child’s Shot/Immunization Records
  • Birth Certificate (9 years and under only)
  • Special Care Plan (asthma, IEP, etc.)
  • Proof of income (one of the following)
  • Current tax return
  • End of year paychecks
  • Written notice from social service agency no more than 30 days old

 

Note: If your child is returning for the school year, the following items do not need to be given to the club unless there is a change in paperwork. Please discuss with your site staff.

Membership Fee: $20 annually  Pay Here

Summer Camp Fee: $120
